About this roleAs a Senior LTSS Service Care Manager at Superior Health Plan, you will have the opportunity to make a meaningful impact on the lives of our members. We are seeking a skilled and compassionate professional to join our team and provide high-quality care to our complex and high-acuity populations.Key Responsibilities

• Care Planning: Develop and continuously assess ongoing long-term care plans and collaborate with the care management team to identify providers, specialists, and community resources needed to address member needs.
• Member Advocacy: Act as a liaison and member advocate between the member, physician, and facilities/agencies to ensure the best possible outcomes.
• Care Coordination: Coordinate and manage care between the member and/or family/caregivers and the care provider team to ensure members receive adequate and appropriate person-centered care or services.
• Quality Improvement: Monitor care plans and member status, identifying trends and areas for improvement to enhance operating performance and quality care in accordance with state and federal regulations.Requirements
  • Graduate from an Accredited School of Nursing or a Bachelor's degree and 4-6 years of related experience.
  • RN - Registered Nurse - State Licensure and/or Compact State Licensure required.
