A company is looking for a Manager, Complex Care Management /
Transition of Care.
Key Responsibilities
Oversees care management documentation to ensure compliance with
regulations
Develops and implements care management policies and procedures
Manages escalated care cases and provides guidance to team
members
Required Qualifications
Master's degree or Graduate from an Accredited School of
Nursing
5+ years of related experience in healthcare leadership or managed
care
Licensed Clinical Behavioral Health Professional or RN as per state
requirements
Experience in transition of care and complex care management
Preference for candidates residing in Oklahoma or central time zone
USA
